DS-160 confirmation page (the online nonimmigrant visa application)
MRV fee payment receipt — the $185 application fee
Both your VAC appointment and consulate interview appointment confirmations
One recent passport-size photo (2×2 inches, white background, taken within 6 months)
Last 6 months of bank statements
